Required forms to start any class<\/h3>\n\n\n\n

If you are over 18 years old, you will complete and submit your required forms on your Scuba Schools International (SSI) account<\/a>.<\/p>\n\n\n\n

As you fill out the medical form, SSI will notify you if you need to download the form and have an\u00a0MD<\/strong>\u00a0or\u00a0DO<\/strong>\u00a0sign it to proceed. If this happens to you, upload the signed document to your portal and notify us, via medical@gypsydivers.com, so that we can remove the \u2018pending\u2019 status.<\/p>\n\n\n\n

If you, or your child, is under 18, we will email you digital copies to electronically fill out.<\/em><\/p>\n\n\n\n

In order to take any of our courses or dive in most locations, you\u2019ll need to have a current Medical Form completed within the year. You can now submit this form on your web-based SSI profile.<\/p>\n
