Required forms to start any class

If you are over 18 years old, you will complete and submit your required forms on your Scuba Schools International (SSI) account.

As you fill out the medical form, SSI will notify you if you need to download the form and have an MD or DO sign it to proceed. If this happens to you, upload the signed document to your portal and notify us, via medical@gypsydivers.com, so that we can remove the ‘pending’ status.

If you, or your child, is under 18, we will email you digital copies to electronically fill out.

Scuba Medical History Form

In order to take any of our courses or dive in most locations, you’ll need to have a current Medical Form completed within the year. You can now submit this form on your web-based SSI profile.

Trip Agreement

The Trip Agreement covers all the safety and logistic basics of going on a Scuba Trip with us!
Once you have been signed up for a trip, please fill this out so we may provide your trip leaders with your important information!
